What is Mohs Micrographic Surgery?

Mohs Micrographic Surgery is the most precise and effective treatment for many types of skin cancer, including basal cell carcinoma and squamous cell carcinoma. It involves removing thin layers of skin one at a time, examining each layer under a microscope until no cancer cells remain. This technique allows for the highest cure rates—up to 99%—while preserving as much healthy tissue as possible. It’s especially useful for skin cancers in cosmetically or functionally important areas like the face, ears, or hands.

Who can perform Mohs Micrographic Surgery?

Mohs surgery should be performed by a fellowship-trained Mohs surgeon—a dermatologist who has completed specialized training in both surgical removal of skin cancer and the microscopic interpretation of skin tissue. This dual expertise ensures both complete cancer removal and optimal cosmetic outcomes. Dr. Cowart is a board-certified dermatologist and fellowship-trained Mohs surgeon dedicated to providing expert, personalized care in a comfortable outpatient setting.

What to expect when having skin cancer treated with Mohs Micrographic Surgery

Mohs surgery is typically performed in our office under local anesthesia. You’ll be comfortably awake throughout the procedure. The process involves removing the visible skin cancer and then carefully examining the tissue under a microscope to check for any remaining cancer cells. This is repeated layer by layer until all the cancer is completely removed. Between each stage, you’ll wait in a private room while we analyze the tissue. Most procedures are completed the same day, and we’ll discuss options for repairing the wound immediately afterward. Our goal is to ensure complete cancer removal with minimal impact to healthy surrounding skin.

How is Mohs Micrographic Surgery different from a standard excision?

Standard excision involves removing the visible skin cancer along with a margin of surrounding healthy skin, which is then sent to a lab for analysis over the next several days. With Mohs surgery, the tissue is examined on-site during the procedure, allowing us to precisely trace and remove any remaining cancer cells while sparing as much normal skin as possible. This results in higher cure rates and smaller scars, especially for cancers in sensitive or cosmetically important areas.
